2. Begin by filling out your personal information at the top of the form, including your name, phone number, and address. Ensure all details are accurate before proceeding.
3. Indicate any public assistance programs you are currently benefiting from by checking the relevant boxes. If you are not receiving benefits from these programs, provide the total number of family members in your household and your total income.
4. For each pet you wish to include in the voucher program, fill out their name, breed/color, sex, and type of animal in the designated sections. Remember, you can include up to eight pets.
5. Read the spay/neuter voucher agreement carefully before signing. Acknowledge the responsibilities and permissions stated in the agreement.
6. Complete the driver license or Florida state ID number and date of birth sections and add your signature, along with today’s date to finalize the application.
7. Before submitting, make sure to include a copy of your valid photo ID and proof of benefit assistance or last year’s tax filing, as required.
8. Finally, save your changes to the completed application and prepare to mail it or hand-deliver it to the Hillsborough County Pet Resource Center.
