7. Submit the verification of work experience by completing the necessary Electrical Experience Verification form (440-2570A) from each of your employers. Include proof if you relied on military experience.
8. Attach a 2” x 2” passport-style photo with your name written on the back. Do not staple the photo to the application.
9. Complete the applicant affidavit section by certifying the accuracy and completeness of your application, signing, and dating the form.
10. Lastly, review the checklist provided at the end of the application to ensure that all required documentation and payments are included before submission.
11. Once you have confirmed that everything is filled out correctly, you can save the changes, download, print, or share the form as necessary.
