9. Enter the date of transfer in MM-DD-YYYY format, which is when ownership is granted.
10. If the property falls under any classification affecting potential penalties, check the corresponding box.
11. Indicate any special circumstances that may have affected the transfer price and provide an explanation if necessary.
12. Complete the income tax withholding section if applicable, specifying if Maine income tax applies.
13. Finally, review the declaration statement, enter the preparer's details if different from the buyer/seller, and ensure all information is correct before submitting the form.
