9. In Section 3, identify the service area for solid waste received. Indicate the source of waste and transport methods for reporting.
10. In Section 4, document the destination of waste sent off-site. Include all necessary details of the facilities or locations where waste was sent.
11. Complete Section 5 if applicable, reporting recyclable and recovered materials received according to the specified guidelines.
12. Continue through the remaining sections regarding unauthorized solid waste, cost estimates, problems encountered, and changes to reports until reaching Section 11.
13. In Section 11, the owner or operator must sign and date the form. Ensure all prior sections are completed accurately before submission.
14. Finally, save the form, ensure all changes are recorded, and either download, print, or share the completed form as needed.
