- If necessary, check off corresponding Plan Requirements based on the listed criteria for fuel burning equipment or fuel oil storage plans.
- Fill in the Burner Specifications by including details like Manufacturer/Trade Name, MEA/BSA Number, Gross Firing Rate of Oil Burners, and Model Number.
- Provide information on Fuel Storage Specifications, noting the quantity of tanks and their respective capacities for different oil grades.
- Complete the Chimney Information section, including materials and your inspection statement regarding construction. Don't forget to sign and date.
- If applicable, the Registered Architect or Professional Engineer should fill in their details and provide a seal and affirmation of correctness.
- After the installation, Oil Burner Installer/Master Plumber details need to be provided, including their affirmation statement and signature upon completion.
- Finally, review all information filled out on the form for accuracy, then save your changes, download, print, or share the Pw1c form as required.
