5. In the agreement information section, specify the contract dates, stating the start and end dates within the allowed one-year term. Fill in the financial information, indicating the amount, frequency, and the square footage of the room(s) to be used.
6. Indicate the days of the week and times of day the facility will be used, and include any other instructions relevant to the application.
7. Document the approval date from the church or school board meeting, ensuring a copy of the meeting minutes is ready for submission.
8. Sign and date the application on both the licensor and licensee signatures lines.
9. Submit the completed application, including all required documents, by emailing it to property@nccsda.com or faxing it to 888.312.0319. Ensure the facility is not used until the application process has been fully completed.
