8. Request a Verification of License in Another State if you currently hold or have held a license in any other state.
9. Include a sealed original Self-Query Report from the National Practitioner Data Bank (NPDB), dated within the last 60 days.
10. Ensure you complete fingerprint requirements using Live Scan or fingerprint cards, as specified based on your residency.
11. Review your application to ensure all fields are filled out accurately, sign and date the application within 60 days, and submit it along with all required documents.
12. After submission, monitor the processing time by the board, which can take up to 30 days. Wait for notification regarding the status of your application.
13. Once processed, you can save changes, download, print, or share the form as needed.
