How to fill out the USCG CG-6000-3 online

The USCG CG-6000-3 form, also known as the Diver/BUD/S Medical Screening Questionnaire, is essential for personnel seeking to participate in diving activities. This guide will assist users in completing the form accurately and efficiently in an online format.

Follow the steps to complete the USCG CG-6000-3 form online.

  1. To begin, click the ‘Get Form’ button to access the CG-6000-3 form and open it in your web browser.
  2. In the first section, enter your name and rank, employee ID (EmplID), date of birth (DOB), present command, branch of service, and the date of submission.
  3. Carefully review the diving medical questions and provide accurate responses. For each question, select 'Yes' or 'No' based on your medical history and current health status. Be honest, as concealment of medical history may lead to disqualification.
  4. If you answered 'Yes' to any diving medical questions, ensure you provide detailed explanations as required on the subsequent pages.
  5. The additional diving medical questions section should then be filled out by a Diving Medical Officer (DMO) or a qualified representative. They will assess previous medical examinations and any necessary documentation.
  6. After filling out all pertinent fields, review your entries to confirm accuracy and completeness.
  7. Final steps include saving your changes, downloading the completed form, printing it for your records, or sharing it as necessary with appropriate parties.

Complete your documentation online for a smoother processing experience.

Physical Requirements: Personnel who volunteer for Navy Diver must be in excellent physical condition. Comfort in the water beyond good swimming ability and no aversion to confinement in close spaces is a must. The ability to perform detailed mechanical tasks under stress and/or adverse conditions is important.

Scuba diving requires heavy exertion. The diver must be free of cardiovascular and respiratory disease (see references, following page). An absolute requirement is the ability of the lungs, middle ears and sinuses to equalize pressure. Any condition that risks the loss of consciousness should disqualify the applicant.

It is quite difficult to become a Navy diver. There are various stages of training involved that test both your mental and physical toughness. Becoming a Navy diver begins with basic training at the Navy's Great Lakes training center.

Swimming, bicycling, and weight training will prepare you for some of the activities at dive school, but ONLY running can prepare your legs for the rigors of the training. You should also become accustomed to running in sand. Notes: For Weeks 8 and beyond, you need not increase the distance of your runs.

The Navy diving physical is rigorous and is required within one year of the initial diver training at the NDSTC. The physical includes a complete neurological examination. Chest X-rays, an EKG, a hearing test, a visual acuity test, a dental examination and several blood tests are part of the physical.
