5. If you need to delete a supplementary card member, fill in their member ID, last name, and first name in the relevant section.
6. Review the Terms and Conditions carefully to ensure your understanding of the application process and the entitlements associated with supplementary cards.
7. Finally, sign and date the form to confirm your agreement to the terms. Ensure all provided information is accurate before submitting.
8. You can save changes, download, print, or share the completed form as needed. Submit the completed form to a Membership Services Counter, or send it via fax or email to ensure your supplementary card is issued promptly.
