2. Begin with the Customer Information section (Section A). Fill in your account number, email address, full name, mailing address, installation address, phone numbers, and indicate your home type by selecting the appropriate option.
3. Next, proceed to Section B for the Contractor/Installer information. Ensure your contractor fills in their company name, address, daytime phone number, email, license number, and installation date.
4. Have your contractor sign and date the Program Compliance section, and indicate whether they will be releasing the rebate.
5. In Section C, your contractor must complete the Equipment Information, providing details about the old unit's manufacturer, model, age, SEER rating, and whether it was a replacement or an upgrade.
6. In Section D, the contractor will fill in details for the new equipment, including the manufacturer and model, AHRI reference number, and specifications regarding the tonnage and type of system installed.
7. After completing all sections, ensure all necessary attachments, such as a copy of the paid invoice and any required calculations, are included.
8. Finally, save your changes. You can choose to download, print, or share the completed form as needed.
