10. If your child is in 3rd grade and was not baptized at St. Michael Church, ensure to attach a copy of the Baptism Certificate as indicated.
11. Provide the name and phone number of an alternate contact person for emergencies in case the church cannot reach you.
12. Note any relevant health issues on the designated area, if applicable, to ensure the church is aware of your child’s needs.
13. Review the form for accuracy and completeness. Once satisfied, sign and date the document to finalize your submission.
