  12. Specify your Immigration Status at Last Arrival in item 24, typically F-1 student, and Current Immigration Status as F-1 in item 25.
  13. Complete item 26 with your SEVIS number, which is found on your I-20.
  14. Select the appropriate eligibility category in item 27: (c)(3)(B) for OPT and (c)(3)(C) for STEM OPT.
  15. If applying for STEM OPT, complete item 28 with your degree name and CIP code.
  16. In Part 3, sign your name in black ink in the Applicant’s Statement and Signature section.
  17. Complete Part 4 if you have used an interpreter. If you filled out the form yourself, skip Part 5.
  18. In Part 6, provide additional details if you have been approved for CPT/OPT in the past or have different SEVIS IDs.
