The birth certificate is the record of the child's birth and is usually created at the hospital when the baby is born. It is not used to establish paternity. If the mother is married when she becomes pregnant or when the baby is born, her husband's name will be put on the birth certificate as the child's father.

You can open a parentage case with the Court. Once the legal parents are established by the Court, follow the procedure on the State Dept. of Public Health website to request that the birth certificate be amended to include the father's name. You and the other parent can sign a Voluntary Declaration of Parentage .

To make a change or correction to a Michigan Birth Certificate you must do so through the State of Michigan. Fees for obtaining a certified copy of a birth certificate are $24 for the first copy, and $7 for each additional copy of the same record.

(4) If the paternity of a child is determined by a court of competent jurisdiction, the name of the father shall be entered on the certificate of birth as found and ordered by the court. The surname of the child shall be entered on the certificate of birth as designated by the child's mother.

A: You must provide: A completed application to change a Michigan birth record. A copy of the birth certificate of the parent with the correct information. OR two (2) documents dated at least five (5) years prior to the date of application which contains the correct information.

Once paternity has been established, a father now has the same rights and obligations as any other parent. These rights include obtaining orders regarding physical and legal custody, having input into decisions concerning the child, spending quality time with the child and being responsible for supporting the child.

A completed form GRO 185 form, signed by both parents, for each child to be re-registered. Both parents are required to attend at the register office for the re-registration.
