8. Proceed to the tuition and registration fees section to acknowledge understanding of the fees associated with enrollment, including registration and weekly rates.
9. Review the parent acknowledgment section. Initial in the provided spaces to indicate your understanding and permission regarding medication administration, media use, and emergency protocols.
10. Finalize by signing and dating the form to confirm your agreement with all terms. Save, download, print, or share the completed form as needed.
