4. Sign the application in the space labeled 'Signature of Applicant.' Make sure to include your Federal ID, Social Security Number, or Driver’s License Number in the designated area.
5. Proceed to have the form notarized. The notary will complete the section that includes the date of attestation and their commission expiration details. Ensure this is completed, as Section 1 must be fully filled out.
6. Section 2 requires the licensed physician's certification. Have a licensed physician, physician assistant, chiropractor, or advanced practice registered nurse sign and date the form, certifying your impairment. Ensure that the type of placard you are applying for is noted.
7. Once all sections are completed and signatures are obtained, review the form for any errors or missing information. Double-check that you have signed where necessary.
8. After reviewing, save your changes. You can then download, print, or share the application form as needed. Make sure to forward the completed form to your County Clerk.
