- The Family Income and Assets section requires you to list the total gross income and payment sources for all household members aged 18 or older. Specify at least one source of income, and if there are none, enter "0" for the Head of Household's income.
- List the total cash value and annual income for any assets owned by household members, such as checking and savings accounts, stocks, and other investment accounts.
- Answer the Eligibility and Preferences questions to determine your eligibility for rental assistance, including questions about wheelchair facilities, veteran status, residence within city limits, and displacement by government action.
- You may also include the optional contact information for someone who can assist you during your tenancy. This is not a requirement but can be beneficial.
- Finally, complete the Certification section by entering the Head of Household's Social Security number and date of birth. Sign and date the application to confirm that all information is true and complete.
- Once you have filled out the form, be sure to save your changes. You can download, print, or share the completed form as needed.

To make the form legally binding, you must sign it in front of a notary public. You must then file your signed and notarized deed with the county office that's in charge of recording property documents. Once the grantee signs the warranty deed, he/she legally has ownership and claim to the property.
