4. Indicate your preferred future contact method for concession entitlement notifications by selecting either 'Post' or 'Email'. If choosing email, provide your email address. Add your contact phone number where prompted.
5. Review Section C, the privacy notice, and ensure your understanding of how your personal information will be managed and protected.
6. Proceed to Section D for the applicant declaration. Read the terms carefully before signing and dating, affirming your consent to the privacy notice and accuracy of the information provided.
7. Before submitting, ensure you have attached a photocopy or scan of your Seniors Card (front and back) and are submitting the application two to three weeks before your intended travel date or have provided a NSW delivery address.
8. Send the completed form either via email to concessions@transport.nsw.gov.au or by post to Transport for NSW, Locked Bag 5085, Parramatta NSW 2124.
