5. Complete Part 4 for authorization. Sign and print your full legal name, provide a daytime telephone number, and the date signed. If there are multiple individuals authorized, repeat the process for each person.
6. Once all sections are completed, review your application for accuracy. Save changes to your document. You can then download, print, or share the completed form as needed.
