11. If applicable, provide the previous dates if you are canceling or changing a prior request.
12. Sign the form as the employee to authorize the request for absence.
13. Obtain an approval signature from your principal or department head.
14. Ensure that the Timekeeper enters the request in Time and Labor with the corresponding Time Reporting Code based on your selected type of leave.
15. Once you have filled out all necessary fields, save your changes, download, print, or share the completed form as needed.
