8. In section 4, check all applicable reasons why it is in the child's best interest to have their name changed.
9. If there is another parent, provide their name and address in section 5. If there is no other parent, check 'None.'
10. If the other parent agrees to the name change, they must sign the form in section 6 in front of an Illinois notary public. If they do not agree, check the relevant option.
11. In section 7, indicate whether there is a second parent or another person not currently with physical custody. If so, fill out and attach the additional form as required.
