If your child was born in a New York hospital not in New York City, and you are signing the Acknowledgment of Paternity after the original birth certificate was filed, the New York State Department of Health will issue you a free birth certificate.

If an unwed father is not listed on the birth certificate, he has no legal rights to the child. This includes no obligation to paying child support and no rights to visitation to custody or child support.

The mother and father can sign an Acknowledgment of Paternity form. This form is usually filled out at the hospital when the child is born but is also available in the local child support office and local birth registrar.

An acknowledgment of paternity will require some basic information including the child's full name, mother's full name, and father's full name. The father's date of birth, address, and Social Security number are also needed. The AOP must be signed and notarized by both parents.
