8. If applicable, confirm if Class 5 Classroom instruction is conducted by another Driver Training School, and attach the necessary agreement letters.
9. Include the names and signatures of all Senior Driving Instructors or Authorized Representatives in the designated area.
10. Designate individuals to contact Alberta Transportation regarding your licence by listing their names and obtaining their signatures.
11. Complete declaration by signing and providing the information of the Owner/Director and Authorized Representative at the bottom of the form.
12. Review the entire application for accuracy, then save your changes. You may download, print, or share the completed form as necessary.
