Follow the steps to complete the form accurately.

  1. Press the 'Get Form' button to access the OK 03PA209E form and open it in your preferred online document editor.
  2. Begin by providing the name of the hospital or entity completing the form. Include the facility code number where applicable.
  3. In the 'Child's Information' section, accurately fill out the child's first name, middle name, suffix, and last name as they appear on the birth certificate. Enter the social security number and the sex of the child (male or female), followed by the date of birth.
  4. Continue to enter the biological mother's information, including her first name, middle name, last name, and maiden name, followed by her date of birth, place of birth, current street address, social security number, city, state, and zip code.
  5. Indicate whether the mother was married at the time of conception or birth. If 'yes', provide the spouse's name and ensure that the spouse or former spouse completes Form 03PA210E, Denial of Parentage.

Paternity can be established if both parents sign an Acknowledgment of Paternity. If the mother is married to a man who is not the natural father of the child, the husband must also sign a Denial of Paternity, 03PA210E. A judge can also establish paternity.

Signing a birth certificate, alone, does not establish paternity in Oklahoma. However, there are certain instances where signing a birth certificate would help a person become recognized as the father.

The child whose parentage has been determined as set forth shall be treated as a child of parents who was married at the time of birth. So this is clearly an intent by the legislature to give equal rights to parents that have a child born to them, whether they're married or not married.

A. Except as otherwise provided in subsection B of this section, a proceeding brought by a presumed father, the mother, or another individual to adjudicate the parentage of a child having a presumed father shall be commenced not later than two (2) years after the birth of the child.

Either the mother or father can go to their local child support office and open a child support case to establish paternity. There is no cost to open a case. If the test is positive, the father will be required to pay the State back for the cost of the testing.

How Long Does a Father Have to Establish Paternity in Oklahoma? A father can establish paternity from birth until the child's 18th birthday.

Paternity means legal fatherhood. Establishing or setting up paternity is the process used to show who is the legal father of a child. When a woman is married, her husband is legally presumed to be the father of the child.

Reasons for Men Without marriage to the mother or name on the birth certificate Oklahoma says fathers have no rights until established by a court order. Child support can still be ordered. If the child is born during a marriage a man and woman have equal rights.

In proven cases of fraud, a father may challenge paternity any time before the child turns 18. Otherwise, anyone who wishes to challenge paternity must do so within 2 years of the child's birth. Please note that proving fraud is difficult, and challenging paternity after a child's second birthday is uncommon.
