Patellar-glide test: This test is used to evaluate the instability. A medial/lateral displacement of the patella greater than or equal to 3 quadrants, with this test, is consistent with incompetent lateral/medial restraints. Lateral patellar instability is more frequent than medial instability.

The patella (kneecap) attaches to the femur (thigh bone) and tibia (shin bone) by tendons. The patella fits into a groove at the end of the femur (trochlear groove) and slides up and down as the knee bends and straightens. Patellar instability occurs when the kneecap moves outside of this groove.

It is thought to be caused by either congenital factors or lateral patellar tracking. Lateral patellar tracking will eventually flatten the trochlear groove leading to patellar instability. Regardless of the cause, trochlear dysplasia results in a loss of the inherent bony stability of the patellofemoral joint.

The BPII is a self-administered questionnaire that can be used to assess the quality of life in patients with patellofemoral instability.

The Banff Patellofemoral Instability Instrument 2.0, known as the BPII 2.0, is a disease-specific patient-reported outcome measure used to measure quality of life in adults and adolescents with lateral patellofemoral instability.

Initial dislocations are generally managed non-operatively with rest and often a knee brace and crutches. After a period of rest, Physical Therapy is usually begun to strengthen the muscles in the knee that help keep the kneecap from sliding out of the groove.

The best test to determine whether a patient is having symptoms from a subluxing or dislocating patella, is the lateral patellar apprehension test. It is performed with the knee flexed to 45° over the side of the examining table.
