3. In the Specialty Training Declaration, confirm your completion of specialty medical training in Radiology or Interventional Radiology. Include the date of completion, ensuring it meets the five-year requirement.
4. If you are an EBIR holder, check the box confirming your credential. If not, fill in the details of two CIRSE Fellows who will support your application, ensuring at least one resides in the EU.
5. For the CVIR Reviewer Database section, indicate whether you agree to be included by checking 'Yes' or 'No'.
6. List five published articles on cardiovascular and interventional radiology topics in the designated area. Ensure these publications are in recognized, peer-reviewed journals.
7. If you have additional articles, provide the necessary details in the optional section.
8. Before submission, review the Application Checklist to confirm all required components are complete, including any necessary letters of support.
9. Once all sections are completed, certify the accuracy of your information with your signature and date.
10. Email the completed application form along with any supporting documents to registration@cirse.org for submission.
