KHSAA Form BK148 AM-Rev 1/12 PNC FOUNDATION/KHSAA SWEET 16 SCHOLARSHIP APPLICATION PNC Foundation will award 32 one-time 1 000 scholarships with one boy and one girl from each basketball region selected as the winner. Scholarships will be based on excellence in three areas -- Academic Achievement Leadership at school and Community Service. To apply complete the form include a school photo and letters of reference and mail it to the KHSAA Attn Butch Cope 2280 Executive Drive Lexington KY 40505. Applications must be received by the KHSAA by Friday Feb. 24 2012. If you do not include all requested information your application will not be considered.
