6. Enter your Job Title, Current User ID if you have one, and respond to whether you currently have a TDOE SSO or Orion account by selecting Yes or No.
7. If applicable, include your Teacher License Number.
8. Input your Work Email Address and Work Telephone Number.
9. Select the specific applications for which access is needed based on your user role. Each application category is listed for your reference.
10. If requesting District Approver access to Attendance Funding, ensure you complete the Notary section on page 2.
11. Justify your access request by listing specific job duties that require access to the requested applications.
12. Attest to the accuracy of the information provided by entering your name and understanding the implications of accessing confidential data.
13. Provide your Supervisor's Name, Title, and ensure the form is submitted by a district representative.
14. Complete the field for Date Form Completed and save your changes, download, print, or share the form as necessary.
