1. Press the ‘Get Form’ button to access the Employee Timesheet. This will enable you to open it for editing.
2. Enter your payroll period in the designated fields, specifying the start and end dates from Sunday to Saturday. Ensure the dates are correct for the service rendered.
3. Fill in your name in the 'Employee Name' section and provide your contact phone number.
4. Indicate the employer you worked for in the related section and specify the case manager's name if applicable.
5. For each service provided, mark the appropriate boxes under 'Tasks (check if performed)'. Ensure this section is filled accurately to reflect your work.
6. Input the date and the times when you started (Time In) and finished (Time Out) your shifts for each day. Remember to circle either A (am) or P (pm) accordingly.
7. If any day falls on a holiday, indicate that by writing “HOLIDAY” on the corresponding day line to be eligible for holiday pay.
8. Summarize the total hours worked for the week in the designated space labeled 'Total Hours for Week'.
9. Both you and your employer or their representative must sign the timesheet, providing your signatures and phone numbers where required.
10. Once the form is completed, review for accuracy, save the changes, and choose to download, print, or share the completed timesheet as necessary.
