5. Complete the driver’s license information. Include your driver’s license number, expiry date, date of birth, and the issuing country.
6. If you are a member of a frequent traveler program, select your program and provide your membership number in the relevant section.
7. Fill in your charge card billing information. Select your card type, enter the card number, and expiry date. Obtain approval from an authorized signatory if you wish to link a company account.
8. Review the insurance section carefully. Indicate your acceptance or rejection of Motor Vehicle Insurance (MVI) and sign where required.
9. Choose your optional coverages such as Personal Accident Insurance (PAI) and Excess Reduction (ER) by initialing your preferred options.
10. Select your car group preference from the options provided.
11. Sign the authorization section, confirming the accuracy of your information and agreeing to the terms and conditions.
12. After completing the form, save your changes and choose to download, print, or share the form as needed.
