4. Indicate your riding by entering the name of the electoral district you reside in.
5. Provide your residential address, ensuring to complete each part including municipality/township and postal code.
6. Enter your home phone number and work phone/cell in the respective fields.
7. Insert your email address; this is required for party communications.
8. If your mailing address differs from your residential address, provide that information in the designated section.
9. Select the duration of your membership by checking the appropriate box with or without an email address. Options include one year or two years, with the corresponding fees.
10. Indicate how you will pay for your membership fee, choosing between paying with your own funds or having an immediate family member cover the cost.
11. If you wish to make a donation to Patrick’s campaign, please enter the amount you would like to donate.
12. Complete the payment details, including the membership fee, donation, and total amount.
13. Select your payment method, choosing from options such as Visa, MasterCard, AMEX, cheque, or cash. Be sure not to mail cash.
14. Fill in the name on the card and enter the card number along with the expiration month and year.
15. Review the declaration statement, ensuring you meet the age requirement and consenting to the collection of your information.
16. Sign and date the application form.
17. Once completed, save your changes, and consider downloading or printing your form for your records. You can also share the form as needed.
