- Complete Part V by marking each applicable taxation district where business was conducted. If applicable, include an X in the box for ‘MULTI’ and attach Form G-75.
- In Part VI, provide totals from previous parts to calculate the total tax due and any penalties. Be sure to calculate the totals in Columns (b) and (c).
- Finalize the form by signing, entering your title, and the date. Provide the daytime phone number for future contact.
- Once you have filled out the form completely, save your changes, and prepare to download, print, or share the completed Form G 49.
