14. Fill in the thirteenth field with the name of your dojo.
15. Record the date and place of your examination in the fourteenth field.
16. Indicate the grade obtained in the fifteenth field.
17. Have an examiner sign in the sixteenth field, confirming your grading.
18. The seventeenth section is for completion by the chief of the Examination Committee and should not be filled out by you.
