How to fill out the Pgsga online
The scored patient-generated subjective global assessment (PG-SGA) is an important tool designed to evaluate a patient's nutritional status. This guide provides a clear, step-by-step approach to completing the PG-SGA online, ensuring accurate information is collected for optimal healthcare outcomes.
Follow the steps to successfully complete the PG-SGA online.
- Press the ‘Get Form’ button to access the PG-SGA form and open it in your editor.
- Begin with the patient identification section. Fill out personal details to the best of your ability. If assistance is needed due to vision or literacy challenges, a trusted individual can help.
- In Box 1, input your current weight in pounds, alongside your height in feet and inches. Then, record your weight from one month ago and six months ago. Remember, use the most recent weight, with the 1-month data being prioritized for scoring.
- Assess your food intake for the past month in Box 2. You will rate changes compared to your normal intake, and select one of the provided options to describe your situation regarding consumed food.
- Box 3 requires identifying any symptoms that have affected your eating. Check all symptoms that apply to you in the past two weeks, ensuring you capture all relevant experiences.
- In Box 4, rate your activity level over the past month. Choose the option that best describes your daily activity, as this impacts your overall assessment.
- Once you have filled out Boxes 1-4, the remaining sections should be completed by your healthcare provider. They will review and score your information based on clinical knowledge.
- After all sections are completed, you can save your changes, download the form, print it for your records, or share it with your healthcare provider.
Complete your PG-SGA online to ensure your nutritional needs are accurately assessed and addressed.
Using subjective global assessment (SGA) involves a comprehensive evaluation of a patient's nutritional status based on clinical judgement. Begin by reviewing the patient's medical and dietary history, then assess their physical condition. This method allows healthcare professionals to identify individuals at risk of malnutrition effectively. Incorporating SGA into your practice can streamline the nutritional assessment process.
