1. To enter the 7th grade, students are required to have one dose of Tdap; because a child must have received at least one dose of the required vaccine to be eligible for conditional admission, Tdap is not included on the Idaho Conditional Admission to School form.

Idaho law allows a parent or guardian to claim an immunization exemption for their child for medical, religious or other reasons. A medical exemption must be completed by a licensed physician.

(a) Immunizations are not mandatory and may be refused on religious or other grounds; (b) Participation in the immunization registry is voluntary; (c) The parent or guardian is entitled to an accurate explanation of the complications known to follow such immunization.

No child shall attend school without proof of immunization status.

Immunization Exemptions A medical exemption must be completed by a licensed physician. It is recommended that exemptions for religious or other reasons be documented on the form provided by Idaho Immunization Program.
