- In Section V, provide the name(s) and Social Security Number(s) of the responsible individual owner(s), partner(s), or corporate officer(s). If there are any changes among these individuals, create a list of those changes. Note that significant business changes may require a new registration form (PA-100) to secure a new Account ID Number. Finally, ensure to sign and date the form, including your daytime telephone number and title.
