10. Indicate the number of copies you wish to receive: official, unofficial, faxed, or a combination thereof.
11. Choose your preferred handling method for delivery: regular mail, priority, or overnight. Provide the recipient's phone number if selecting overnight delivery.
12. Once all fields are completed, review your request for accuracy. Save any changes, then download, print, or share the form as needed.
13. Send the completed form back to the Office of the Registrar via fax, email, or standard mail as provided in the instructions.
