4. In the Certification Basis section, specify the CASR Part applicable. Indicate if it is a Foreign certification from a recognised country, provide the Type Certificate Number if available, or note if it’s a Type Acceptance Certificate. If issued by Australia or falls under another category, specify accordingly.
5. For Maintenance, complete the Certificate of Approval Holder/CAMO Number section by entering the Name and Location.
6. In the Component Details section, list the Model Designation for the Basket and Burner. For Cylinders, enter the Manufacturer name, Serial Numbers, and Hydrostatic Test dates for each cylinder.
7. Fill in the Instrument Pack details, including the Manufacturer, Model Designation, and Serial Number.
8. Complete the CASA Delegate/Authorised Person section by entering the CASA Delegate's Name, Instrument of Appointment Number, and Authorised Person's signature and date. Include ARN as applicable.
9. Once all sections are completed, review the form for accuracy. Save your changes, and choose to download, print, or share the completed AU CASA 369 form.
