4. List any winter and spring sports the student plans to participate in by writing them in the designated spaces.
5. If there have been any changes to personal information since the original submission, update the current home address and telephone numbers for both the parent/guardian and the student.
6. Next, check for any changes to emergency contact information and provide updates where needed, including names, relationships, addresses, and phone numbers.
7. In the Supplemental Health History section, answer each question regarding the student's health, circling 'Yes' or 'No' as applicable. Make sure to explain any 'Yes' answers at the bottom of the form.
8. Both the student and parent/guardian must sign the document, certifying that all information is accurate and complete, including dates.
9. Once completed, review the entire form for accuracy. Save changes to the document, and choose to download, print, or share the form as needed.
