How to fill out the TN Advance Care Plan online
The TN Advance Care Plan is a vital document that allows individuals to express their healthcare preferences in situations where they may no longer be able to communicate. This guide provides a clear and supportive approach to completing the form online, ensuring your wishes are understood and respected.
Follow the steps to complete your advance care plan effectively.
- Press the ‘Get Form’ button to access the TN Advance Care Plan document and open it for editing.
- Begin filling out Part 1 by identifying your agent, the individual responsible for making healthcare decisions on your behalf. Provide their name, address, relationship to you, and multiple contact numbers, including home, mobile, work, and any other phone.
- If necessary, designate an alternate agent in case your primary agent is unable or unwilling to serve. Fill out the same details as in step 2 for this person.
- Select when the authority of your agent becomes effective by marking either: ‘I give my agent permission to make health care decisions for me at any time’, or ‘I do not give such permission (this form applies only when I no longer have capacity)’. Make your choice clear by checking the appropriate box.
- In Part 2, express your wishes regarding quality of life by marking ‘yes’ or ‘no’ for each provided condition. The conditions include permanent unconsciousness, permanent confusion, dependency in all activities of daily living, and end-stage illnesses.
- Indicate your treatment preferences for each relevant condition. For each treatment option listed, mark ‘yes’ for treatment you want or ‘no’ for treatment you do not want, including CPR, life support, treatment of new conditions, and tube feeding/IV fluids.
- Provide any additional instructions in Part 3 about matters such as hospice care and burial arrangements. Attach extra pages if needed.
- In Part 4, specify your wishes regarding organ donation. Choose from the options provided and mark your preference clearly.
- Finally, sign the document in Part 5. Ensure your signature is witnessed either by two competent adults (Block A) or by a notary public (Block B), following the specified requirements.
- Once completed, save your changes, and consider downloading, printing, or sharing the finalized document as needed. Make sure to provide copies to your physician and those you have appointed as your healthcare agent.

An example of an advance directive order is a living will that specifies your desire to receive comfort care only if you are terminally ill. In this document, you might state that you do not wish to receive aggressive treatments like ventilation or feeding tubes. Such instructions form part of your TN Advance Care Plan, making it clear to healthcare professionals and your loved ones exactly what care you want. By providing this clarity, you help avoid confusion or conflict in difficult situations.
