1. Click the ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin by entering the student’s name in the designated field at the top of the form.
3. Review the list of required courses, which include Math, Language Arts, US History, Earth Science, PE/Health, and Computer Literacy. These courses must be selected for the student's schedule.
4. Move on to the elective choices section. Here, you will select your preferred electives. Remember that you must request a combination of electives to complete a full year. You can choose either two semester electives or one full-year elective.
5. Fill in your primary elective requests in the designated boxes. Make sure to number them according to your preference.
6. In the alternate elective requests section, add up to five additional elective choices. These will be used if your primary requests are not available.
7. If you are interested in electives marked with an asterisk (e.g., Advanced Art or Teacher/Office Aides), remember that an application is required. Ensure that the necessary application forms are obtained from the front office or your counselor.
8. Finally, provide your signatures in the respective signature fields for both the parent and student.
9. After you have completed all sections, review the form for accuracy. Once confirmed, you can save any changes as needed, and proceed to download, print, or share the completed form.
