Follow the steps to successfully fill out the order form.

1. Press the ‘Get Form’ button to access the KS Buddy Poppy Order Form and open it in your available editor.
2. Fill in the date of your order at the top of the form. This is important for processing and tracking your request.
3. Enter your Post or Auxiliary Number and the city associated with your order to ensure proper identification of your organization.
4. In the 'Mail to & Payable to' section, confirm the address is correct for the Department of Kansas VFW, which is PO Box 1008, Topeka, KS 66601-1008.
5. Indicate the date of the poppy sale to help in the scheduling and tracking of your order fulfillment.
6. Complete the total amount of your order and specify the number of poppies you wish to order, noting the minimum order requirement of 100 and allowable increments.
7. Provide the amount included with your order and check off the check number if applicable. Include any balance due at this stage.
8. In the shipping or mailing address section, provide a complete address where the poppies should be sent. Remember that orders of 500 or more cannot be shipped to P.O. Boxes.
9. Enter your name and Post/Auxiliary membership number for accountability in the ordering process.
10. Add your email address and daytime phone number to facilitate communication regarding your order.
11. Once you have filled out all the necessary fields, review your entries for accuracy before proceeding. Finally, save any changes, download, print, or share the form as needed.
