Follow the steps to successfully complete your order form

1. Click ‘Get Form’ button to access the HiSET Official Practice Test Order Form and open it for completion.
2. Choose the desired practice test from the list provided. Each test is identified by a title and an item number. Indicate the quantity you would like to order for each selected test.
3. Calculate the total cost for each practice test by multiplying the quantity by the unit price of $10. Add these amounts together for a subtotal.
4. Include the flat shipping fee of $5.50 and, if applicable, your state tax in the total cost section.
5. Fill out the shipping information section with the recipient's name, title, institution, address, city, state, ZIP code, phone number, and email address.
6. If the billing information differs from the shipping information, complete the billing section with the necessary details.
7. Select your method of payment and provide any required payment information. If paying by credit card, ensure that the cardholder name, number, and expiration date are filled out correctly (for fax or mail only).
8. Sign the form to authorize the payment, and ensure all required fields are completed accurately before submitting.
9. Submit your completed order form and payment via mail, fax, or email to ETS Order Services at the provided addresses.
10. Keep a copy of your completed form for your records, and allow 4–6 weeks for processing and shipping.
