- Select your type of business ownership by checking the appropriate box: Individual, Corporation, Partnership, State/federal government, or Non-profit organization.
- If you selected 'Corporation', indicate the date and state of incorporation.
- List the owners, partners, or corporate officers with their respective social security numbers, names, titles, and locations.
- Detail the number of qualified motor vehicles operated both within and outside of Illinois.
- Check the types of fuels used in the qualified motor vehicles by selecting from the provided options.
- List jurisdictions where you maintain bulk fuel storage. Additional sheets may be attached if necessary.
- Complete your decal order by calculating the total cost based on the number of decals needed, multiplying this by $3.75 per set.
- Sign the application, ensuring that the signer is an authorized individual identified in step 3. Provide printed name, date, title, and telephone number.
- Finally, mail the completed application to the address specified for the Motor Fuel Use Tax Section of the Illinois Department of Revenue.
