5. Indicate the ceremonial support you are requesting. Check the relevant boxes for services such as guest speaker, band, catafalque party, or marching contingent.

7. Select your preferred service (Navy, Air Force, Army, ADF Cadets), or leave it blank if you do not have a preference.
8. Respond to the questions regarding past ADF support and whether your event will require a COVID Safe Plan.
9. If you wish to request a ceremony flypast, indicate 'yes' and specify the type of aircraft along with the fly-past time and location details.
10. Read and agree to the acknowledgement section regarding the possibility of support withdrawal and ensure you include your name, title, position, and date.
11. Once you have filled out all the sections, save your changes, download a copy, or print the completed form. Finally, submit the request form via email to the appropriate state or territory JOSS office.
