The purpose of this report is to tell the court why your service as guardian is ending and what has happened to the protected person and the protected person's assets since you filed your last report.

Citation: Alaska Stat. When there has been transfer of legal custody or appointment of a guardian and parental rights have not been terminated by court decree, the parents shall have residual rights and responsibilities.

This form allows a temporary one-year delegation of powers by a parent or guardian to a 3rd party. The form gives all the powers of a parent or guardian to the named person for up to one year.

When a family member or friend is not able or willing to serve in this capacity, the court will appoint a private professional guardian/conservator, or the Public Guardian. A private professional guardian/conservator will provide these services for compensation, usually charging on an hourly basis.

Guardianship – A legal arrangement where the court appoints a person or institution as a guardian to make decisions for an incapacitated person, and/or a child – decisions about housing, medical care, legal issues, financial issues and services. The individual being cared for in the guardianship is called the “Ward”.
