14. List the Driver Information, including the total drivers operating CMVs along with those holding a valid Commercial Driver's License.
15. Respond to the question regarding the revocation status of your USDOT number by indicating yes or no.
16. If applicable, certify compliance as a Passenger Carrier by responding appropriately to the certification question.
17. Enter the names and titles of sole proprietors, partners, or corporate officers responsible for the application.
18. Finish the form with the Certification Statement, ensuring it is signed and dated by an authorized individual. The signature must match the printed name provided.
19. After reviewing the completed form for accuracy, save changes, download a copy for your records, and print or share the form as needed.
