- Complete the Hazardous Materials section by indicating which materials your company handles as either a Carrier or Shipper.
- Provide detailed information on the number of Commercial Motor Vehicles operated, categorized by ownership type, including various types of transport vehicles.
- List the Driver Information, including the total drivers operating CMVs along with those holding a valid Commercial Driver's License.
- Respond to the question regarding the revocation status of your USDOT number by indicating yes or no.
- If applicable, certify compliance as a Passenger Carrier by responding appropriately to the certification question.
- Enter the names and titles of sole proprietors, partners, or corporate officers responsible for the application.
- Finish the form with the Certification Statement, ensuring it is signed and dated by an authorized individual. The signature must match the printed name provided.
