- In Section 4, for retired members only, provide the names of beneficiaries for the lump-sum death benefit. Complete the same information as in previous sections, including the designation of primary or contingent and their relationship to you.
- Finally, in Section 5, sign the form as the member and provide the date. If applicable, ensure your spouse or registered domestic partner also signs in Box B, acknowledging the beneficiary designation.
