- Explain how you have been affected by the issue and what resolution you would like from the financial services provider.
- Read and understand the declaration statement. Confirm that all information provided is accurate and give your consent for the Ombud to handle your personal information.
- Sign and date the form for yourself and any additional complainants before proceeding with submission.
