Please complete the application and return it to our office along with a copy of your driver's license. There will be a $250.00 (refundable) deposit which is required upfront, and a $45.00 (non-refundable) transfer fee on first month’s bill. The deposit must be paid by cash, money order, or credit card over the phone (5% processing fee.
