The tuberculosis (TB) blood test, also called an Gamma Release Assay or IGRA, is a way to find out if you have TB germs in your body. The TB blood test can be done instead of a TB skin test (Mantoux). There are two kinds of TB blood tests: QuantiFERON®-TB.

An induration of less than 5 millimeters (mm) is a negative result. If you have symptoms or know you've been exposed to someone with TB, a doctor may recommend you get another test. If the induration is at least 5 mm, it is considered positive in people who: have had recent contact with a person with TB.

Any applicant diagnosed with infectious tuberculosis disease who needs treatment must receive a classification of Class A TB and is not cleared for travel until successful treatment completion, regardless of the diagnostic criteria.

Patients in hospitals or nursing homes, children under 4 years of age, and patients who use injection drugs with ≥10 mm induration should be considered positive. Anyone 4 years and older with a low risk for TB infection is considered positive with ≥15 mm induration. (See Accurate interpretation.)

The reference range of the induration is 0-15 millimetres at an increment of 5 mm, 10 mm and 15 mm. An induration size of more than 15 mm is considered positive in a normal patient. A swelling of less than 4 mm is considered negative.

Positive TB blood test: This means that the person has been infected with TB bacteria. Additional tests are needed to determine if the person has latent TB infection or TB disease.

TB infection happens in 4 stages: the initial macrophage response, the growth stage, the immune control stage, and the lung cavitation stage. These four stages happen over roughly one month.

TB-5 - TB suspect Diagnosis pending (a patient should not be in this class for more than 3 months).
